Finding the right health coverage can be confusing—but you don’t have to do it alone. Heartland Health Services’ trained Community Health Workers are here to guide you through your options, answer your questions, and help you apply for coverage that fits your needs and budget. Illinois Medicaid

Free or low cost health coverage for eligible, low-income Illinois residents, including families, children, pregnant women, elderly, and individuals with disabilities.

Am I Eligible?

 

Medicare

Federal health insurance for people 65 and older, and younger people with certain disabilities. We help with enrollment and plan comparisons.

Moms & BabiesIllinois Moms & Babies is a Medicaid program that provides comprehensive healthcare coverage for pregnant women and their babies up to one year after birth. This program ensures access to essential prenatal, delivery, and postpartum care, as well as doctor visits, hospital services, and prescriptions at little to no cost. At Heartland Health Services, our team is here to assist eligible patients with enrollment, helping them receive the care they need for a healthy pregnancy and baby.

 

 

 

                               

 

 

All KidsIllinois All Kids is a state program that provides comprehensive healthcare coverage for children, regardless of family income or immigration status. This program ensures that kids have access to doctor visits, dental care, vision services, prescriptions, and more at little to no cost. Heartland Health Services is committed to helping families navigate the enrollment process so their children can receive the quality care they need to grow up healthy and strong.

 

 

 

Family Planning Program/Presumptive EligibilityThe Family Planning Program (FPP) provides confidential, low-cost, or free reproductive health services to individuals of all ages. This program covers essential care such as birth control, annual exams, STI testing and treatment, pregnancy counseling, and more. At Heartland Health Services, we support patients by offering access to these vital services, ensuring they receive the care and education needed to make informed decisions about their reproductive health.

 

Family Planning Presumptive Eligibility (FPPE) provides immediate, temporary coverage for family planning services to eligible individuals. This program ensures quick access to the same essential services as the Family Planning Program. The FPPE application process does not take immigration status into account, making it accessible to more individuals in need. What to bring with you

 

 

Different health coverages have different eligibility qualifications. For some coverages, you must meet certain citizenship or residency requirements, be an Illinois resident, have a social security number, and meet certain financial requirements.

 

To complete your application as quickly as possible and to ensure a smooth processing time, please bring the following with you to your scheduled appointment.

 

Social security card

Valid driver's license/state ID or passport

Proof of address

Proof of income & financial resources

 

 

If you don't have the documents listed above, or have questions, please let the Outreach worker know when you schedule your appointment.
