Heartland Health Services prioritizes your health and safety and understands the specific needs for physical distancing. We believe in a patient-centered model of care, in all available forms. With our telehealth services, you will be able to see your clinician and get your check ups, in the comfort of your own home. 

Telehealth appointments can be requested for established Heartland Health Services patients. Telehealth appointments are available Monday through Friday, during regular health center hours. Please fill out the Contact Us form below to request a telehealth appointment.

What is telehealth?

Telehealth is the use of video conferences, telephone calls, or images transmitted via email or fax to facilitate clinical health care and education.

How does telehealth operate?

Our patient care representatives will help you make either a phone or video call appointment. Video visits will be completed through Zoom (a free video communications service). Phone appointements may be conducted with any accessible phone. 

How do I check my telehealth information?

We recommend you use our patient portal, MyChart, to check your appointment status, see past medical records, and message your providers. 

How do I get to the patient portal, MyChart?

If you have not created an account for MyChart, we recommend you make an account by visitng our patient portal or going online to the website: hhsilmychart.com/hhsil/.
