Schedule an Appointment by calling (309)680-7600. If you haven't already done so, schedule an appointment with one of our healthcare clinicians to determine if you qualify for Heartland's 340B Pharmacy Program.
Bring your Prescriptions: Bring your prescription along with this temporary pharmacy card to one of our partner pharmacies listed below.
Inform the Pharmacy: Let the pharmacy staff know that you are a Heartland 340B patient to obtain potential discounts on your prescription medications.
