Heartland Health Services now partners with ChartSpan to provide eligible Medicare patients with personalized care management services that help you stay healthy, manage chronic conditions, and get the support you need between appointments.

Whether you have questions about your medications, need help scheduling appointments or simply want guidance from a healthcare professional, we're here for you.

Care Management is a Medicare-supported program designed to help eligible patients better manage their health between visits with their provider. 

When you enroll, you'll have access to a dedicated care team that works alongside your Heartland Health Services provider to help you achieve your health goals and navigate your care with confidence.

Care management helps you stay healthier by providing additional support between office visits. 

Those who participate often experience: 

• Better management of chronic health conditions
• Improved communication with their healthcare team
• Few unnecessary emergency room visits
• Reduced hospitalizations
• Greater confidence in managing their health
• Easier access to healthcare resources and support

This program is available to eligible Medicare patients receiving care through Heartland Health Services. 

If you qualify, a ChartSpan Care Management Representative may contact you to discuss the program and answer any questions you have.
